PRECLINICAL STUDY Breast duct anatomy in the human nipple: three-dimensional patterns and clinical implications

چکیده
Background The anatomy of the nipple has become clinically relevant. Diagnostic techniques access the breast through nipple ducts and surgeons offer nipplesparing mastectomy. There is variation in the number of ducts reported and little is known about the spatial location of ducts, their size, and their relationship to orifices on the surface. Methods Nipple specimens were taken from 129 consecutive mastectomies. Each was sectioned coronally into 3 mm blocks and one section was prepared from each block. The number of ducts and cross-sectional areas of nipple and duct ‘bundle’ were recorded. Three nipples were sectioned at 50 lm intervals and digitally reconstructed in three dimensions. Results The median number of ducts was 23 (interquartile range 19–28). Reconstructions and summary data from 25 nipples show a central duct bundle narrowing to form a ‘waist’ as the ducts enter breast parenchyma. A three-dimensional reconstruction focusing on one nipple tip demonstrated 29 ducts arising from 15 orifices. Beneath the skin, most ducts are very narrow, gradually becoming larger deeper within the nipple. Conclusions This work demonstrates that many ducts share a few common openings onto the surface of the nipple, explaining the observed discrepancy between number of ducts and of orifices. Neither duct diameter nor position predicts whether a duct system will terminate close to the nipple or pass deeper into the breast. These new insights into nipple anatomy will be of use in considering the reliability of a ductal approach to diagnosis and in planning nipple-sparing mastectomy.

Results of the radioguided surgeries among the patients with pathological nipple discharge
Introduction: Nipple discharge is the third most common complaint of the breast diseases following the breast mass and the breast pain. The majority of the lesions causing pathological nipple discharge (PND) consist of non-palpable breast lesions (NPBLs). When non-operative diagnostic tools are inadequate to distinguish the diagnosis, the pathological duct should be removed to ...

BACKGROUND Increasing interest in the intraductal approach to the breast has necessitated revisiting the anatomy of the breast. METHODS Using six different complementary in vivo and in vitro approaches, the authors determined the number, distribution, and anatomic properties of the ductal systems of the breast, which extend from the nipple orifices to the terminal duct lobular units. RESULT...
متن کاملTerminal duct lobular units are scarce in the nipple: implications for prophylactic nipple-sparing mastectomy: terminal duct lobular units in the nipple.
BACKGROUND The use of nipple-sparing mastectomy (NSM) for both breast cancer treatment and risk reduction is increasing. There is no randomized data comparing nipple-sparing mastectomy with standard mastectomy techniques. There is evidence to suggest that ductal and lobular breast cancer arises in the terminal duct/lobular unit (TDLU). This study was undertaken to determine whether TDLUs exist ...

OBJECTIVE The main function of the breast is to produce milk for offspring. As such, the ductal system, which carries milk from the milk-secreting glands (alveoli) to the nipple, is central to the natural function of the breast. The ductal system is also the region in which many malignancies originate and spread. In this study, we aimed to assess the feasibility of manual mapping of ductal syst...
متن کاملEffect of aloe vera gel on nipple sores
Objective: It is estimated that 80-90% of breastfeeding women experience nipple soreness. In this study we evaluated the effect of Aloe Vera gel compared with breast milk on nipple sores. Methods: This clinical trial study was performed in the neonatal ward of the Ghaem Hospital in Mashhad (Iran) from June 2009 to December 2009. Mothers were divided randomly into two groups: Aloe Vera applica...
